About Our Founder
Colleen Van Horn is the Chief Executive of Innovative Healthcare Consultants, Inc. Throughout her years working as a registered nurse, she specialized in adult intensive care and cardiac nursing as well as a hospital case manager. She became a Certified Case Manager and also holds a certification as a Public Health Nurse.
 Ms. Van Horn is recognized locally as an expert in the field of geriatric case management. She is a member of the National Association of Professional Geriatric Care Managers and Case Management Society of America. She has been a speaker at numerous organizations involved in eldercare and volunteers time to assist the elderly in a variety of philanthropic activities.
Company History
Innovative Healthcare Consultants, Inc. opened it's doors in June of 1997. The rapid changes in the healthcare maze was leaving many elderly confused and defenseless in regard to their healthcare rights. For this reason the company was founded. The company became committed to assisting individuals and their families with the challenges of aging. The goal was to maintain the well being, independence and dignity of the older family member while striving to ensure the highest quality and most cost-effective healthcare is achieved.
Today, our staff of RN's complement and assist each other with challenging cases. Innovative Healthcare care managers have advanced nursing degrees and come from backgrounds such as geriatric-psych, cardiac ICU, rehabilitation, home health and insurance/worker's compensation.
For our clients who need personal care, Innovative Healthcare now employs part-time and full-time companion caregivers. Many of our caregivers are Certified Nursing Assistants, Home Health Aids and Medical Assistants. These caregivers are supported, trained and supervised by our case managers. The case manager and caregiver work as a team with members of the family to provide the highest level of service. Innovative Healthcare caregivers provide many services including special assistance with dementia and behavior problems. For a complete list of services, please refer to our Caregivers section.
We also have an office staff of four, including a staffing coordinator, insurance specialist, business manager and administrative assistant.
The corporate office is in Fallbrook, in No. San Diego County, California. The service areas are San Diego County and So. Riverside Counties.

Innovative Healthcare Consultants is part of a large community of elder services in San Diego County, including for profit, non-profit, and government agencies. In providing services for our clients, or for others seeking services, we often provide appropriate referrals to other agencies. We make these referrals based on professional judgment to assist an individual in finding the best service to meet her or his needs. At no time does Innovative Healthcare Consultants solicit, accept, or pay fees for referrals.
Regarding mailing lists, client lists and prospective client contact information, we respect your privacy and will not share your name, address, or other personal information with other companies or agencies, nor will we sell information to a marketing clearinghouse.
