They come from a wide range of medical, clinical, social, and rehabilitation fields and are exceptionally resourceful.

Teresa Gardner, RN, CMC
Teresa has over 40 years of nursing experience. She received her RN from Holy Name School of Nursing in New Jersey. She then completed her Adult Nurse Practitioner program from State University of NY. She is certified in MICN, BCLS, ACLS, PALS, TNCC and CATN. She has worked for many years as an RN in emergency room medicine and as a RN for hospice, home health and health clinics. She offers patients education and supportive care in the S. Riverside areas and Fallbrook. |

Teresa Hueners, RN
Terri has her associate degree in Nursing and is working on her BSN and case management certification. She has been an RN for 15 years. She is also a CCRN (critical care RN), specializing in cardiovascular pulmonary rehabilitation and cardiac rehabilitation. Terry worked at Tri City hospital for many years and assisted with many advocacy and educational programs for the public. Terry services the north coastal and north inland San Diego county areas. |

Linda Maker RN, CMC
Linda has been a nurse for over 25 years. She has an extensive background in acute care nursing, home health, hospice care, adult psychiatric nursing, Alzheimer’s day care centers, and behavioral health nursing. She has organized and taught continuing education classes to CHHA’s/CNA’s. She also is a certified recovery room nurse. Linda provides services in San Diego's N. County Inland area and S. Riverside county. |

Doreen Salerno RN, MSN, BSN, PHN, CMC
Dori has a Master’s of Nursing Administration with a Bachelor’s of Science in Nursing. She has over 35 years of Registered Nursing experience. Her experience includes research for cancer/psychiatric, home health field nursing, surgical nursing, crisis psychiatric nursing, political and administrative intern and a program director for a gero-psychiatric unit. Dori is also a member of the National Association of Professional Geriatric Care Managers. She provides services in the San Diego coastal area. |

Roberta Smith, RN, BSHSA, MSAEd, Ph.D., CMC
Roberta has 34 years experience as a registered nurse, with geriatrics as the focus. She has served as the supervisor or head nurse in area hospitals, mostly in medical units or psychiatric treatment facilities. She taught continuing education for nurses at 3 colleges and in private industry before spending 10 years as a maritime medical missionary in the Pacific. Her greatest interest is home health and hospice care. Her education includes health service administration and adult education degrees. She received her RN from Riverside Community College and her BS in Health Service Administration from the University of Phoenix. She also received her degrees from Columbia Pacific University for Master of Science and Doctor of Philosophy. She will be assisting clients in Fallbrook and the surrounding areas. |

Beth Walter RN, CCM
Beth received her associate degree in nursing from Ventura College. She is a certified case manager (CCM) and a member of The Case Management Society of America. Beth’s nursing experience includes charge nurse and case manager for psychiatric hospitals, along with an extensive background coordinating care for capitated contracts for inpatient and SNF services. She has worked with the Kaiser system, collaborating care with all interdisciplinary team members. Beth services the Inland area of San Diego County. |
